You should not have to re-tell your child's history.
The reason parents repeat themselves at every appointment is that nobody is keeping the whole picture. Each provider keeps the slice they are paid for, and the join between them lives only in your head.
The file is the join. It is the same record your Family Lead works from, the same one the clinician updates, and the same one you can open on your phone in a school meeting.
We keep it because being able to see the work is the only way you can hold us to it.
Six things, kept current.
The Independence Plan
Goals in plain English, intensity, settings, projected timeline and review dates. Every version kept, so you can see what changed and when.
Session data
What was worked on, how your child did against each goal, and the practitioner's note. Added the same week, not at the end of term.
Reports from everyone
Including reports written by clinicians and schools we have nothing to do with, if you want them in one place.
Reviews and decisions
Every review, and every decision with the reason it was made. Including the decisions you disagreed with, recorded as such.
Hours and cost over time
Support hours by month against the plan's projection, so a reduction that was promised and never happened is visible to you, not only to us.
Who has seen it
An access log. You grant and revoke access to the school, a clinician or a family member, and you can see who opened what.
The file is yours. Not ours.
Export it free, any time
PDF for reading and CSV for the data, on request, at no cost. There is no export fee and no exit fee, because charging a parent to retrieve their own child's history would be indefensible.
Free on the way out, too
Especially on the way out. If you leave us, we send the complete file to whoever you name, in a format the next provider can actually open.
You control who sees it
Access is granted by you, per person, and revoked by you. A school gets what you decide a school gets. Nothing is shared with a third party for marketing, ever.
Corrections are yours to demand
If something in the record is wrong, you can require it to be corrected. If we disagree, your objection is recorded in the file next to the entry.
